Below is the registration form for 2010.  If your institution a law enforcement organization, please enter $0 as the membership fee, otherwise enter $150.   If you are a financial institution, please fill out the section marked Financial Institutions Only - Reward Assessment.  The rules for the total assessment amount are located at the bottom of the form.

2010 Reward Assessment-

If your F.I. has 140 branches, your assessment for 2010 is $2,000.00.

If your F.I. has 2 branches, your assessment for 2010 is $40.00.

Your institution's Main Office Branch or any branch office in the same building as that of your main office should not be assessed. Remote ATM's are covered under the reward program but not separately assessed.
